Lars and the real girl: I think that the main character has a mental disorder because his behavior is aberrant; normal people do not consider a doll to be his or her espouse. Though being different from the normal standard does not necessarily consider whether one has a disorder or not, but in this case, there is definitely a problem with how the character behaves. I think that his friends are well aware of his problem and are trying to help him by mingling with Lars and the doll, creating a comfortable environment. This is definitely affecting the friends around Lars because they are assimilating into this environment.
Anne also has a psychological disorder, but it isn’t as devastating as Lars’s. We can infer that Anne has a disorder because though her behavior does not affect her, it affects how her family and students at her school treat her as a person who is anomalous.
